In the event any crown, bridge, inlay or onlay used in any dental treatment performed in our practice fails, within five (5) years from the date of service, we agree to repair or replace them free of charge, subject only to the following limitations and conditions:

1. You must complete the treatment plan within the agreed upon timeframe. You must also follow the directions given to you by your doctor for the optimum care of your restorations.
2. You must maintain good oral health by attending all of your individualized hygiene visits (At the recommended intervals depending upon your periodontal health status.)
